Your insurance policy is a contract between you and your insurance company. We cannot guarantee payment of your claims by your insurance. While most policies under in-network plans are processed as in-network, some plans fall outside the network. We will do our best to determine your benefit level and coverage during the courtesy benefit check we offer at your initial visit or upon scheduling.
Please bring your current medical insurance card(s) to your initial visit and notify us if there is a chance in your insurance coverage. Payment is due at time of service according to your quoted plan benefits. If there is a discrepancy between what your insurance quoted and what the EOB indicates, we will notify you verbally at your appointment or via mailed statement.
